Therapist notes are therapist-authored notes with their own visibility rules, separate from the client-owned cards and wording clients create.

Separate from client-owned cards
- Therapist notes do not change a client’s card title, wording, sharing state, or archive state.
- Client-owned card content remains the client’s wording and should not be overwritten by therapist interpretation.
- Use notes to prepare, clarify questions, or record workspace context without treating shared cards as a complete record.
Client-visible notes
- Therapist notes are private by default unless an explicit visible-to-client control is available and enabled.
- Client-visible notes should be written with care because clients may read them directly.
- If visibility is unclear, pause before adding sensitive note content and confirm how the current screen works.
Safety and documentation boundaries
- Notes should support session preparation and shared understanding, not replace clinical documentation where the therapist has separate professional obligations.
- Do not include crisis or emergency assumptions in notes expecting StephX to notify someone automatically.
- Therapists remain responsible for professional judgement, mandated reporting requirements where applicable, and emergency or crisis protocols outside StephX.
